Easy access – for your patient and for you
Our MRI referral and safety form is available here. The completed form can be faxed or emailed back. There is no wait time. Any urgent or same day requests, however must be made by telephone.

Patients will receive a CD with their exam images. Physicians and health professionals can also request a CD or have access to the exam via our secure on-line access.

We will accept a referral form from any public or private MRI facility.
